The Patient Health Questionnaire-9 (PHQ-9) is a clinical instrument for screening, diagnosing, monitoring, and measuring the severity of depression. Comprising nine questions, it aligns with the criteria for major depressive disorder in the Diagnostic and Statistical Manual of Mental Disorders (DSM).

Response Options: Each question is rated on a scale of 0 to 3, based on the frequency of the symptom over the past two weeks:

0: Not at all
1: Several days
2: More than half the days
3: Nearly every day
